De acordo com o próprio Tolkien, Sauron tinha uma forma física na terceira idade:

...in a tale which allows the incarnation of great spirits in a physical and destructible form their power must be far greater when actually physically present. ... Sauron should be thought of as very terrible. The form that he took was that of a man of more than human stature, but not gigantic. ~The Letters of J. R. R. Tolkien

A pergunta que estou pedindo é: ele sobrevive ao anel sendo destruído? Ele é visto morrendo? Ou ele simplesmente não é visto de todo?

O espírito ou essência de Sauron sobreviveu, ele já não tem ou terá força para fazer o mal novamente, a menos que toda a sua essência tenha sido enviada para Mandos, caso em que as portas do salão abrem todas as apostas.

Morgoth / Melkor, o Ainu que o corrompeu, ainda existia, e eventualmente ele retornaria para enfrentar Tulkas (Thor como Demi god, a Vala), Fëanor (criador dos Silmarils) e Turin Turumbar (figura hercúlea do primeira idade).

Turim e sua espada negra Gurthang deveriam acabar com a existência de Melkor e com ele todo o mal, mas isso estava literalmente no fim dos tempos.

O próprio Sauron não foi visto no filme, apenas seu Grande Olho. Quanto à sua destruição: do capítulo O Último Debate de O Retorno do Rei Gandalf diz do Um Anel

"If it is destroyed, then he will fall; and his fall will be so low that none can foresee his arising ever again. For he will lose the best part of the strength that was native to him in his beginning, and all that was made or begun with that power will crumble, and he will be maimed for ever, becoming a mere spirit of malice that gnaws itself in the shadows, but cannot again grow or take shape. And so a great evil of this world will be removed."

Sauron sobrevive à destruição do anel, mas ele é impotente para agir na Terra-média novamente. No entanto, no final original de O Silmarillion Tolkien escreveu que Sauron surgiria durante o Dagor Dagorath ou "Última Batalha e Dia da Perdição" quando seu antigo mestre Morgoth quebra a Porta da Noite e destrói o sol e a lua.
