A primeira instância está em Câmara dos Segredos , onde a turma questionou Binns sobre a Câmara. Binns claramente deixa todos os 3 nomes de seus alunos errados.
No começo, ele é informado do sobrenome de Hermione:
'Miss— er—?'
'Granger, Professor.'
[...]
'My subject is History of Magic,' he said in his dry, wheezy voice. 'I deal with facts, Miss Granger, not myths and legends.'

Assim, nas duas primeiras instâncias, é o professor: o professor Binns, que esquece os nomes de seus alunos. Provavelmente devido ao fato de que ele era muito velho. A outra é no caso mais notório de Slughorn, que não se incomoda em lembrar os nomes dos alunos "não talentosos".
